Bring blank copies of W-9 and ensure schools have updated forms. Bring a new form to each school if you have changed your name or address within the last year or if it's been more than 2 years since you have last been paid by the school.

Timer/Scorekeeper Responsibilities
Keep a copy in your bag to give to timer/scorekeeper to review if they do not know their responsibilities
Tied games will go into a shoot out. Be sure to have a copy of the recording sheet on hand in case the home school doesn't.

This is the rating sheet used at Play Day. Use it for reference when filling out your peer evaluations.
Please use blocks to limit your travel. If you can't leave the office until 3 PM, then put a partial block in until 3 PM.
Do not use travel limits to partially block days. Make sure your Postal Code is correct with where you would be leaving on each day.
